Day Program at our Break-Away Centre

At Monte Care we don’t just think of you – your carer (family or friend) is not far from our minds, and so, we offer a Day Program to provide a day’s break to your carer but equally important is our aim to provide you with a structured social and activities program at our Break-Away Centre in Kogarah.

Our structured social and activities program is designed by our in-house Community Services Manager against the background of a fun and relaxed environment where you can socialise, relax and enhance or learn new skills in a comfortable, and friendly surroundings. Our Day Program is offered once a month or more frequently depending on demand. Pick-up and drop-off can be arranged with Monte Care.

Our aged care services are designed to help older Australians, 65 and over, live independently in the comfort of their own homes.

We provide personalised support tailored to your needs, whether it’s daily living assistance or long-term care through the Support at Home Program (formerly referred to as Home Care Package). We understand the importance of maintaining your lifestyle and independence, and our dedicated team ensures that you receive the highest quality care, respecting your choices and preferences every step of the way.

Services for keeping you well

We have designed our personal care to help you with your grooming and maintenance of your personal hygiene. These include daily tasks like showering, dressing, shaving, oral hygiene, ensuring comfort and dignity every step of the way. Our caring and trained support workers offer assistance while respecting individual preferences and routines.

We understand that maintaining a balanced diet can be challenging especially for elderly individuals. We want you to eat well to sustain your energy. Our goal is to provide nutritious, delicious  meals for your individual dietary needs and preferences.

We understand that health issues can be challenging and we are here to help. We will work with you to manage your personal conditions to promote independence at home.

Our community nursing care is provided by qualified nurses who assist with a range of medical assistance, including complex wound care. We’re here to ensure your health and wellbeing are managed in the comfort of your own home.

Services for keeping your home liveable

We provide practical household support to help keep your home running smoothly. Whether it’s cleaning (vacuuming, dusting), beddings, laundry, ironing, or general household tasks, our team ensures that you or your loved one can live comfortably and safely at home.

Fall prevention must be a top priority. We can assist in certain ways to create a safer environment for you such as inspecting and ensuring that your home is well lit and without clutter to avoid trip hazards.

Maintaining a safe and tidy outdoor space is important, which is why we offer simple gardening and yard maintenance services. From mowing the lawn, pruning, to tidying up flower beds, we’ll help ensure your outdoor areas are pleasant and well kept.

Services for getting you out and staying social

Our social support services aim to reduce isolation and help you stay engaged with the community. Whether it’s companionship, attending social outings, or participating in hobbies, we will work with you or your loved one to stay connected, active, and happy. We have a Day Program at our Break-Away Centre with this in mind.

We believe that mental stimulation is good for you. We have designed activities to enhance mental engagement in a group or one-on-one setting.

Getting around shouldn’t be a challenge. With our travel and transport services, we ensure you can attend medical appointments, social activities, or other engagements with ease. Our reliable and safe transport service is here to get you where you need to go.

Services for getting temporary help

We understand the importance of providing breaks for carers. We can provide you with short term round the clock care or constant supervision and assistance in your own home when your carer needs to take a break.

If you need assistance at home while recovering from illness or surgery, our skilled support workers and nurses can provide you with compassionate care and right level of care during your recovery period all of which are aimed at promoting your well-being and a smooth transition back to your daily life.

Getting started

Getting started with Monte Care is easy, simply make an enquiry or referral by using our forms and our team will guide you through the process to ensure the right services are tailored to meet your needs or those of your loved ones.

Have not yet applied for NDIS support funding?

These simple steps may help you.

STEP 1: Check your General Eligibility Requirements

  • Age – Are you aged between 9 and under 65 years old?
  • Residency Status – Are you an Australian citizen or permanent resident?
  • Disability – Do you have a disability impairment (intellectual, cognitive, neurological, sensory, physical, or psychosocial) that is permanent and significant? If yes, do you usually need disability-specific supports to complete your daily life activities?

STEP 2: Complete the NDIS Request Form (Form) if you meet all requirements in STEP 1

  • You may access the Form by downloading it from the NDIS website at www.ndis.gov.au or by calling the NDIA on 1800 800 110. Read, understand and complete the Form carefully.

STEP 3: Ask your Treating Professional, for example, your GP or Specialist, to complete the relevant sections of the Form

  • Your GP or Specialist (e.g. Psychiatrist, Neurologist) will need to provide evidence of your primary disability and any other disabilities you may have, and a confirmation if your disability is permanent.

STEP 4: Check your Application Form for completeness

  • The Form has a Checklist attached to it to help you check the Application Form for completeness.
  • Ensure all required proofs in the form of documentary evidence are included with the completed Application Form, such as proof of age and residence, disability, and any other additional proofs required.

STEP 5: Submit the Completed/Signed NDIS Access Form (with all proofs) via any of the following means:

  • Email: NAT@ndis.gov.au – this is the quickest, or
  • Mail: GPO Box 700, Canberra, ACT 2601, or
  • Hand delivery in person: Take it to your local NDIA office

STEP 6: Prepare for a planning conversation

  • You will be asked about how and where you want to have the planning conversation, such as in your house or over the telephone.
  • You will be asked about your daily life. Some questions you may be asked are:
    • What you’d like to do in the future, how NDIS can help you with what you want to do.
    • Your goals
    • Residence and living arrangements
    • How you move around your home and community
    • Who currently supports you. For example, your family, friends or service providers
    • What self-care support you need.
    • If you use or need equipment, technology, or devices, also known as assistive technology
    • What social and recreation activities you like to do now or in the future
    • If you need help to build friendships or connect with your family
    • If you would like to work or study now or in the future
    • What support you need to build your skills and do more things.

STEP 7: Implementation Meeting

  • Once your Plan is approved, you will be contacted for an implementation meeting.
  • The meeting is to help you understand the plan and what supports can be bought with the funding.
  • You will be provided with a copy of your NDIS plan after it is approved.

Assistance with daily personal activities

We provide the support you need for your daily personal activities such as personal hygiene (showering, bathing, toileting, dressing, grooming), eating, attending appointments, managing medication.

Our goal is to help you maintain your independence and dignity while ensuring you receive the care and support you need. Our support workers focus on creating a comfortable and respectful environment, tailored to your unique preferences and routine, making daily life smoother and more manageable.

Assistance with high intensity daily personal activities

If you have complex health or care needs, we offer specialised support to assist with high intensity daily personal activities.

This includes services such as administering medication, managing complex health conditions, using medical devices, and providing care in emergencies. Our highly trained staff are experienced in delivering care that meets these needs with expertise, compassion, and attention to detail, ensuring you feel safe and well cared for at all times.

Community nursing care

Monte Care offers comprehensive community nursing care, designed to manage your complex health needs within the comfort of your own home.

Our qualified nurses provide a range of medical services including wound care, health monitoring, stoma care, administering medications, support after a stay in hospital and post operative care, and coordinating complex care plans. With a focus on convenience, safety, and professional oversight, our nursing services will allow you to receive expert medical care.

Household tasks

We will assist with essential household tasks that you are not able to complete to contribute to your having a safe and comfortable living environment.

Our services include cleaning, simple gardening or yard maintenance, laundry/linen, ironing, home maintenance, meal preparation and delivery, and general household organisation, allowing you to focus on other aspects of your life while we assist you with the upkeep of your home.

Our team ensures that homes are well maintained, reducing stress and supporting independent living.

Development of daily care and life skills

Our development of daily life skills program is designed to empower you to build independence and self confidence in everyday life.

Whether it’s learning about social or physical activity skills, how to manage finances, meal preparation, communicating effectively, managing own personal hygiene, we will provide you with practical support and guidance to help you gain the skills you need to achieve your goals, live more independently, and feel more in control of your life.

Support coordination

Navigating the NDIS can be complex, but our support coordination services make it easier.

We work closely with you to help you understand your NDIS plan, connect with appropriate services, and coordinate your care effectively. Our team is  dedicated to ensuring that you get the most out of your NDIS funding, providing personalised strategies and ongoing support to manage services that best suit your needs and goals.

Assistance with employment & education

We will support you in seeking employment or education opportunities.

Our services include practical assistance with job applications, vocational training, and enrolment in educational courses. Whether it’s securing a job or furthering education, we provide guidance and support and help with resources and tools at every stage.

Interpreting and translating

Apart from having an in-house accredited interpreter, members of our team are from diverse linguistic backgrounds.

Our team helps bridge language barriers. This service ensures that you and your family can fully engage in your care and decision making, providing peace of mind and clarity in every interaction

Assistance with travel & transport

We provide safe and reliable travel and transport services, helping you attend medical appointments, social outings, work, or community activities.

Our transport solutions are designed to promote independence and ensure you can be up and about and can stay connected with your community. Whether accompanied or travelling solo, you can rely on us for punctual, secure, and comfortable travel that meets your specific needs.

Innovative community participation

Our innovative community participation services focus on helping you engage in meaningful social, recreational, and community-based activities.

We encourage individuals to build social skills, develop friendships, and take part in activities that bring joy and fulfilment to their lives. From joining local groups to attending events, our services are tailored to ensure that participants are actively involved in their communities, enriching their personal and social wellbeing.

Daily life tasks in a group or shared independent living (SIL)

We will assist and support you with your daily living tasks in a group or shared living environment.

We focus on developing and enhancing your independence and social interaction while ensuring that daily personal care, household tasks, and group living arrangements are managed smoothly.

Participation in community, social and civic activities

Our community participation service is for you to be able to develop an active and meaningful engagement within your local community to give you a sense of belonging and inclusion.

We work closely with you to identify activities and events that you are interested in or may develop an interest in, whether it’s books, art, theatre, music, sports, community fairs, shopping trips, or any appointment. Our aim is to support you in meeting people, building lasting relationships, developing new skills, and enjoying a fulfilling social life within your community.

Group and centre-based activities

We have a range of social inclusion and activities program specially designed to assist you to lead a full and happy life.

Our aim is to be able to bring people together in a supportive, friendly, and comfortable environment for socialisation and personal development in a group setting. Activities may include recreational, educational, or wellness programs all of which are aimed at enhancing your physical, mental, and emotional well-being. We have developed group activities for our Day Program at our Break-Away Centre in Kogarah.

Have not yet applied for an Aged Care Support at Home Package?

These simple steps may help you.

STEP 1: Check your eligibility

  • You must be over 65 years or older; or
  • You must be 50 years or older if you identify as an Aboriginal or Torres Strait Islander person,

    and are in need of services to support you to stay at home.

STEP 2: Make a request for an Aged Care assessment for a Support at Home Package with My Aged Care via:

  • Online: myagedcare.com.au – the application takes 10 – 15 minutes
  • Phone: 1800 200 422 – there are support options available if you need assistance on the call.
  • In person – make an appointment with an Aged Care Specialist Officer (ACSO) at a Services Australia centre or by calling 1800 227 475.

STEP 3: Complete the application form

You may complete the application yourself or with the help of another person as your supporter.

3.1 What will I be asked when I apply?

You will be asked some simple questions about your current situation and your goals to check if you meet the requirements for an assessment.  As such, you will be asked about:

  • your health
  • how you’re managing at home
  • any support you’re currently receiving
  • what could help you live well (e.g. to maintain a healthy lifestyle, do more of the things you enjoy or get around more easily).


If you meet the requirements for an assessment, you will be asked for a few more details to complete your application.

Once your application is complete, you will get a call from an assessment organisation to discuss your needs and, depending on the outcome of the discussion, arrange a date for the assessment that is carried out in your home or at other locations or via telehealth.

STEP 4: Prepare for your assessment meeting

The assessor will ask to see some documents.  Make sure that you have the following documents and information:

  • your Medicare card and one other form of ID – such as driver’s license, healthcare card, passport or DVA card
  • a copy of any referrals from your doctor  
  • contact details for your GP or other health professionals  
  • information on any support you receive
  • information you already have about aged care services that you may want to discuss.


Your assessor will ask you about:

  • what support you already have, and if it will continue
  • your lifestyle and health, including any concerns you may have
  • how you’re going with completing daily tasks and activities around the home
  • if you have problems with your memory
  • any issues relating to home and personal safety
  • family, community, and cultural activities
  • speaking to your GP or other health professionals.


4.1 Your Support Plan

Your assessor will work with you to develop your support plan

Your support plan sets out the care and services that will best help you. It may also contain things that help you regain confidence and the ability to resume daily activities – it records what’s discussed and agreed during your assessment such as:

  • your strengths
  • your difficulties
  • your goals
  • what you would like to achieve
  • what preferences you have for your services.


STEP 5: Your assessment result

If you are approved for Support at Home, the assessment organisation will send you (and, where relevant, your registered supporter) by post or email a Notice of Decision letter. You can also view your support plan by logging into your My Aged Care Online Account

5.1 Notice of Decision Letter

The Notice of Decision letter contains:

  • information about the services you’re approved to receive
  • approval for any short-term support, if applicable
  • the reasons and evidence supporting the assessor’s decision
  • your rights to have the decision reviewed if you disagree with it
  • a copy of your Support Plan. 


5.2 Support at Home Priority System

You will be placed in the Support at Home Priority System until you are allocated with a Support at Home your budget or funding. This means you will be placed on a waitlist (based on your needs and circumstances).

The wait time for funding depends on the date of approval, your Priority Category, based on the information collected during your aged care assessment, and if you’re approved for a short-term pathway. The higher your priority category, the less time you will have to wait to start receiving your approved funding.

The Support at Home Priority System allocates funding based on the following categories:

  • Urgent
  • High
  • Medium
  • Standard


If you’re actively seeking care when you’re approved, you will be automatically placed in the Support at Home Priority System. Your status will be set as ‘seeking services’ and you will receive funding as soon as it becomes available.  

In some instances, you may get access to AT-HM funding while waiting for Support at Home ongoing funding.  

5.3 Assistive Technology – Home Modifications (AT-HM)

If you have been approved to receive funding through the AT-HM scheme, it uses separate priority systems to allocate funding:

  • immediate
  • high
  • medium
  • standard.


5.4 Interim Funding

When wait times for ongoing funding in the Support at Home Priority System are longer than expected, you may be assigned interim funding, which means 60% of your approved funding will be allocated, allowing you to receive the most critical services to help you remain living at home. The rest of your budget is assigned as soon as funding is available. 

STEP 6: Budget or Funding Approval

You will receive a letter telling you that you’ve been allocated Support at Home funding, including information about any short-term pathway approvals.  

You have 56 days from the date of your letter to enter into a service agreement with your chosen provider and start services. You can contact My Aged Care for a 28-day extension if you need more time to find a suitable provider.

If you do not enter into a service agreement and start services within that period, your funding will be withdrawn. It will then be allocated to the next person on the Support at Home Priority System. You can re-join the Support at Home Priority System by calling My Aged Care.

STEP 7: Your Support at Home package and your Referral Code

A referral code is your unique reference number for receiving services. You give the referral code to your chosen provider. With the code, a provider can view your client record and support plan. They can also accept the referral and start organising services for you. If you lose your code, you can log in to your My Aged Care Online Account to see it.

If you want to change providers, you will need to reactivate your referral code. You can do this in your My Aged Care or by calling us on 1800 200 422.

  • You will receive a letter from My Aged Care when you are assigned a Support at Home Package.
  • Your letter will also provide you with a Referral Code to activate your Support at Home Package funding – you will need to provide your Referral Code to your Services Provider to facilitate the activation of your Support at Home package.


STEP 8: Raising concerns about your assessment outcome

You have the right to raise any concerns about your assessment outcome, such as priority or classification levels. Your letter will have further information about how you can appeal the decision.
